A clear, careful route into support

Arranging care can feel complicated. Our approach is to make each stage understandable, gather the right information and agree expectations before support starts.

1. Initial conversation

We start by listening. Tell us about the person needing support, current circumstances, what matters most and any immediate concerns.

2. Assessment and suitability

Where appropriate, we assess needs, risks, preferences, communication, routines, medicines or health-related considerations, and the outcomes the person wants to achieve.

3. Care planning

If we can meet the needs safely, we agree the care plan, visit or support arrangements, responsibilities, communication routes and review expectations.

4. Start, monitor and review

Support begins once arrangements are clear. We then monitor how the plan is working and review it when needs or circumstances change.
